Summary:
Fixes T13433. Currently, "Login Screen Instructions" in "Auth" are shown only on the main login screen. If you enter a bad password or bad LDAP credential set and move to the flow-specific login failure screen (for example, "invalid password"), the instructions vanish.
Instead, persist them. There are reasonable cases where this is highly useful and the cases which spring to mind where this is possibly misleading are fairly easy to fix by making the instructions more specific.
Test Plan:
- Configured login instructions in "Auth".
- Viewed main login screen, saw instructions.
- Entered a bad username/password and a bad LDAP credential set, got kicked to workflow sub-pages and still saw instructions (previously: no instructions).
- Grepped for other callers to `buildProviderPageResponse()` to look for anything weird, came up empty.

Summary:
Ref T7707. This ends up being sort of complicated: to support 100x100 images in T4406, we need to scale small images //up// so they look OK when we scale them back down with `background-size` in CSS.
The rest of it is mostly straightforward.
Test Plan:
- Did an OAuth handshake and saw a scaled-up, scaled-down profile picture that looked correct.
- Used Pholio, edited pholio, embedded pholio.
- Uploaded a bunch of small/weird/big images and regenerated all their transforms.
- Uploaded some text files into Pholio.
- Grepped for removed methods, etc.

Summary:
Ref T1536. Currently, we have separate panels for each link/unlink and separate controllers for OAuth vs LDAP.
Instead, provide a single "External Accounts" panel which shows all linked accounts and allows you to link/unlink more easily.
Move link/unlink over to a full externalaccount-based workflow.
Test Plan:
- Linked and unlinked OAuth accounts.
- Linked and unlinked LDAP accounts.
- Registered new accounts.
- Exercised most/all of the error cases.
