c43618a3a8 Add "Move Left" and "Move Right" to dashboard tab panels
Summary: Depends on D20474. Ref T13272. Provide an easy way to rearrange tabs on a tab panel, by moving them left or right from the context menu.

Test Plan: Moved tabs left and right. Tried to move them off the end of the tab list, no luck.

ea6c0c9bde Refine the "Mangled Webserver Response" setup check
Summary:
Ref T13259. In some configurations, making a request to ourselves may return a VPN/Auth response from some LB/appliance layer.

If this response begins or ends with whitespace, we currently detect it as "extra whitespace" instead of "bad response".

Instead, require that the response be nearly correct (valid JSON with some extra whitespace, instead of literally anything with some extra whitespace) to hit this specialized check. If we don't hit the specialized case, use the generic "mangled" response error, which prints the actual body so you can figure out that it's just your LB/auth thing doing what it's supposed to do.

Test Plan:
  - Rigged responses to add extra whitespace, got "Extra Whitespace" (same as before).
  - Rigged responses to add extra non-whitespace, got "Mangled Junk" (same as before).
  - Rigged responses to add extra whitespace and extra non-whitespace, got "Mangled Junk" with a sample of the document body instead of "Extra Whitespace" (improvement).

bfe8f43f1a Use "QUERY_STRING", not "REQUEST_URI", to parse raw request parameters
Summary:
Fixes T13260. "QUERY_STRING" and "REQUEST_URI" are similar for our purposes here, but our nginx documentation tells you to pass "QUERY_STRING" and doesn't tell you to pass "REQUEST_URI". We also use "QUERY_STRING" in a couple of other places already, and already have a setup check for it.

Use "QUERY_STRING" instead of "REQUEST_URI".

Test Plan: Visited `/oauth/google/?a=b`, got redirected with parameters preserved.

c5772f51de Fix Content-Security-Policy headers on "Email Login" page
Summary:
In D20100, I changed this page from returning a `newPage()` with a dialog as its content to returning a more modern `newDialog()`.

However, the magic to add stuff to the CSP header is actually only on the `newPage()` pathway today, so this accidentally dropped the extra "Content-Security-Policy" rule for Google.

Lift the magic up one level so both Dialog and Page responses hit it.

Test Plan:
  - Configured Recaptcha.
  - Between D20100 and this patch: got a CSP error on the Email Login page.
  - After this patch: clicked all the pictures of cars / store fronts.

308c4f2407 Fix "AphrontRequest->getRequestURI()" for requests with "x[]=1" parameters in the URI
Summary:
Ref T13250. See PHI1069. This is a small fix for `getRequestURI()` currently not working if the request includes "x[]=..." PHP-flavored array parameters, beacause they're parsed into arrays by `$_GET` and `setQueryParams(...)` no longer accepts nonscalars.

Instead, just parse the raw request URI.

Test Plan: Visited `/search/hovercard/?phids[]=X`, no more fatal. Dumped the resulting URI, saw it had the right value. Tried `?phids[]=x&x=1&x=1&x=1`, saw the parameters correctly preserved.

187356fea5 Let the top-level exception handler dump a stack trace if we reach debug mode before things go sideways
Summary:
Depends on D20140. Ref T13250. Currently, the top-level exception handler doesn't dump stacks because we might not be in debug mode, and we might double-extra-super fatal if we call `PhabricatorEnv:...` to try to figure out if we're in debug mode or not.

We can get around this by setting a flag on the Sink once we're able to confirm that we're in debug mode. Then it's okay for the top-level error handler to show traces.

There's still some small possibility that showing a trace could make us double-super-fatal since we have to call a little more code, but AphrontStackTraceView is pretty conservative about what it does and 99% of the time this is a huge improvement.

b9a1260ef5 Improve top-level fatal exception handling in PHP 7+
Summary:
Depends on D20137. Ref T13250. Ref T12101. In versions of PHP beyond 7, various engine errors are gradually changing from internal fatals or internal errors to `Throwables`, a superclass of `Exception`.

This is generally a good change, but code written against PHP 5.x before `Throwable` was introduced may not catch these errors, even when the code is intended to be a top-level exception handler.

(The double-catch pattern here and elsewhere is because `Throwable` does not exist in older PHP, so `catch (Throwable $ex)` catches nothing. The `Exception $ex` clause catches everything in old PHP, the `Throwable $ex` clause catches everything in newer PHP.)

Generalize some `Exception` into `Throwable`.

Test Plan:
  - Added a bogus function call to the rendering stack.
  - Before change: got a blank page.
  - After change: nice exception page.

e03079aaaa Try harder to present display/rendering exceptions to the user using standard exception handling
Summary:
Ref T13250. When exceptions occur in display/rendering/writing, they currently go straight to the fallback handler. This is a minimal handler which doesn't show a stack trace or include any debugging details.

In some cases, we have to do this: some of these exceptions prevent us from building a normal page. For example, if the menu bar has a hard fatal in it, we aren't going to be able to build a nice exception page with a menu bar no matter how hard we try.

However, in many cases the error is mundane: something detected something invalid and raised an exception during rendering. In these cases there's no problem with the page chrome or the rendering pathway itself, just with rendering the page data.

When we get a rendering/response exception, try a second time to build a nice normal exception page. This will often work. If it doesn't work, fall back as before.

b3e4743ad7 Read POST data sightly earlier in request startup
Summary:
On instances, the "SiteSource" (for site config) pretty much copy-pastes the "read POST data" block because it needs to make some decisions based on POST data when handling inbound mail webhooks.

Move the upstream read a little earlier so we can get rid of this. Now that this step is separated and must happen before the profiler, there's no reason not to do it earlier.

Test Plan: POSTed some data across pages without issue, will remove duplicate code in upcoming change.

a24e6deb57 Read "$_POST" before hooking the profiler, and remove "aphront.default-application-configuration-class"
Summary:
Ref T4369. Ref T12297. Ref T13242. Ref PHI1010. I want to take a quick look at `transaction.search` and see if there's anything quick and obvious we can do to improve performance.

On `secure`, the `__profile__` flag does not survive POST like it's supposed to: when you profile a page and then submit a form on the page, the result is supposed to be profiled. The intent is to make it easier to profile Conduit calls.

I believe this is because we're hooking the profiler, then rebuilding POST data a little later -- so `$_POST['__profile__']` isn't set yet when the profiler checks.

Move the POST rebuild a little earlier to fix this.

Also, remove the very ancient "aphront.default-application-configuration-class". I believe this was only used by Facebook to do CIDR checks against corpnet or something like that. It is poorly named and long-obsolete now, and `AphrontSite` does everything we might reasonably have wanted it to do.

Test Plan: Poked around locally without any issues. Will check if this fixes the issue on `secure`.

b8cbfda07c Track MFA "challenges" so we can bind challenges to sessions and support SMS and other push MFA
Summary:
Ref T13222. See PHI873. Ref T9770.

Currently, we support only TOTP MFA. For some MFA (SMS and "push-to-app"-style MFA) we may need to keep track of MFA details (e.g., the code we SMS'd you). There isn't much support for that yet.

We also currently allow free reuse of TOTP responses across sessions and workflows. This hypothetically enables some "spyglass" attacks where you look at someone's phone and type the code in before they do. T9770 discusses this in more detail, but is focused on an attack window starting when the user submits the form. I claim the attack window opens when the TOTP code is shown on their phone, and the window between the code being shown and being submitted is //much// more interesting than the window after it is submitted.

To address both of these cases, start tracking MFA "Challenges". These are basically a record that we asked you to give us MFA credentials.

For TOTP, the challenge binds a particular timestep to a given session, so an attacker can't look at your phone and type the code into their browser before (or after) you do -- they have a different session. For now, this means that codes are reusable in the same session, but that will be refined in the future.

For SMS / push, the "Challenge" would store the code we sent you so we could validate it.

This is mostly a step on the way toward one-shot MFA, ad-hoc MFA in comment action stacks, and figuring out what's going on with Duo.

Test Plan:
  - Passed MFA normally.
  - Passed MFA normally, simultaneously, as two different users.
  - With two different sessions for the same user:
    - Opened MFA in A, opened MFA in B. B got a "wait".
    - Submitted MFA in A.
    - Clicked "Wait" a bunch in B.
    - Submitted MFA in B when prompted.
  - Passed MFA normally, then passed MFA normally again with the same code in the same session. (This change does not prevent code reuse.)

9481b9eff1 Allow "Can Configure Application" permissions to be configured
Summary:
Ref T13216. See PHI980. Currently, each application in {nav Applications > X > Configure} has a "Can Configure Application" permission which is hard-coded to "Administrators".

There's no technical reason for this, there just hasn't been a great use case for unlocking it. I think when I originally wrote it our protections against locking yourself out of things weren't that great (i.e., it was easier to set the policy to something that prevented you from editing it after the new policy took effect). Our protections are better now.

The major goal here is to let installs open up Custom Forms for given applications (mostly Maniphest) to more users, but the other options mostly go hand-in-hand with that.

Also, in developer mode, include stack traces for policy exceptions. This makes debugging weird stuff (like the indirect Config application errors here) easier.

Test Plan:
  - Granted "Can Configure Application" for Maniphest to all users.
  - Edited custom forms as a non-administrator.
  - Configured Maniphest as a non-administrator.
  - Installed/uninstalled Maniphest as a non-administrator.
  - Tried to lock myself out (got an error message).

81a7c9ac43 Remove the execution time limit (if any) before sinking HTTP responses
Summary:
Ref T12907. At least part of the problem is that we can hit PHP's `max_execution_time` limit on the file download pathway.

We don't currently set this to anything in the application itself, but PHP often sets it to 30s by default (and we have it set to 30s in production).

When writing responses, remove this limit. This limit is mostly a protection against accidental loops/recurison/etc., not a process slot protection. It doesn't really protect process slots anyway, since it doesn't start counting until the request starts executing, so you can (by default) //send// the request as slowly as you want without hitting this limit.

By releasing the limit this late, hopefully all the loops and recursion issues have already been caught and we're left with mostly smooth sailing.

We already remove this limit when sending `git clone` responses in `DiffusionServeController` and nothing has blown up. This affects `git clone http://` and similar.

(I may have had this turned off locally and/or just be too impatient to wait 30s, which is why I haven't caught this previously.)

Test Plan:
  - Poked around and downloaded some files.
  - Will `curl ...` in production and see if that goes better.

7622f6afcc Fix excessively severe CSP URI error during first-time setup
Summary:
See D19394. Currently, during first-time setup before you configure "phabricator.base-uri", we may attempt to generate a setup page, try to generate a CSP header for it, and fail to access the environmental config. This causes a too-severe error page ("configure phabricator.base-uri") instead of preflight guidance (like "can't connect to MySQL").

Instead, treat this more like "security.alternate-file-domain" and just bail on CSP if we can't fetch it.

Test Plan: On a fresh (non-explodey laptop) install with critical setup errors (no MySQL installed yet), loaded Phabricator. Before: error about phabricator.base-uri. After: more helpful guidance about installing/configuring MySQL.

fb4ce851c4 Add a PDF document "rendering" engine
Summary:
Depends on D19251. Ref T13105. This adds rendering engine support for PDFs.

It doesn't actually render them, it just renders a link which you can click to view them in a new window. This is much easier than actually rendering them inline and at least 95% as good most of the time (and probably more-than-100%-as-good some of the time).

This makes PDF a viewable MIME type by default and adds a narrow CSP exception for it. See also T13112.

Test Plan:
  - Viewed PDFs in Files, got a link to view them in a new tab.
  - Clicked the link in Safari, Chrome, and Firefox; got inline PDFs.
  - Verified primary CSP is still `object-src 'none'` with `curl ...`.
  - Interacted with the vanilla lightbox element to check that it still works.

Summary:
Ref T4340. Some "Register/Login" and "Link External Account" buttons are forms which submit to third-party sites. Whitelist these targets when pages render an OAuth form.

Safari, at least, also prevents a redirect to a third-party domain after a form submission to the local domain, so when we first redirect locally (as with Twitter and other OAuth1 providers) we need to authorize an additional URI.

Test Plan: Clicked all my registration buttons locally without hitting CSP issues.

ab579f2511 Never generate file download forms which point to the CDN domain, tighten "form-action" CSP
Summary:
Depends on D19155. Ref T13094. Ref T4340.

We can't currently implement a strict `form-action 'self'` content security policy because some file downloads rely on a `<form />` which sometimes POSTs to the CDN domain.

Broadly, stop generating these forms. We just redirect instead, and show an interstitial confirm dialog if no CDN domain is configured. This makes the UX for installs with no CDN domain a little worse and the UX for everyone else better.

Then, implement the stricter Content-Security-Policy.

This also removes extra confirm dialogs for downloading Harbormaster build logs and data exports.

Test Plan:
  - Went through the plain data export, data export with bulk jobs, ssh key generation, calendar ICS download, Diffusion data, Paste data, Harbormaster log data, and normal file data download workflows with a CDN domain.
  - Went through all those workflows again without a CDN domain.
  - Grepped for affected symbols (`getCDNURI()`, `getDownloadURI()`).
  - Added an evil form to a page, tried to submit it, was rejected.
  - Went through the ReCaptcha and Stripe flows again to see if they're submitting any forms.

9658249ac5 Add "Referrer-Policy: no-referrer" to standard HTTP headers
Summary:
Ref T4340. Some browsers respect this header and referrers are a plague upon the earth.

Also, upgrade "never" to the more modern value "no-referrer".

Test Plan:
In Safari, Firefox and Chrome, disabled `rel="noreferrer"` on links and generated a normal link to an external site. Then clicked it and checked if a referrer was sent.

  - Safari respects meta only, but "no-referrer" is fine.
  - Firefox respects both (either the header or meta tag are individually sufficient to stop referrers).
  - Chrome respects both (same as Firefox).

dba4c4bdf6 Emit a "Content-Security-Policy" HTTP header
Summary:
See PHI399. Ref T4340. This header provides an additional layer of protection against various attacks, including XSS attacks which embed inline `<script ...>` or `onhover="..."` content into the document.

**style-src**: The "unsafe-inline" directive affects both `style="..."` and `<style>`. We use a lot of `style="..."`, some very legitimately, so we can't realistically get away from this any time soon. We only use one `<style>` (for monospaced font preferences) but can't disable `<style>` without disabling `style="..."`.

**img-src**: We use "data:" URIs to inline small images into CSS, and there's a significant performance benefit from doing this. There doesn't seem to be a way to allow "data" URIs in CSS without allowing them in the document itself.

**script-src** and **frame-src**: For a small number of flows (Recaptcha, Stripe) we embed external javascript, some of which embeds child elements (or additional resources) into the document. We now whitelist these narrowly on the respective pages.

This won't work with Quicksand, so I've blacklisted it for now.

**connect-src**: We need to include `'self'` for AJAX to work, and any websocket URIs.

**Clickjacking**: We now have three layers of protection:

  - X-Frame-Options: works in older browsers.
  - `frame-ancestors 'none'`: does the same thing.
  - Explicit framebust in JX.Stratcom after initialization: works in ancient IE.

We could probably drop the explicit framebust but it wasn't difficult to retain.

**script tags**: We previously used an inline `<script>` tag to start Javelin. I've moved this to `<data data-javelin-init ...>` tags, which seems to work properly.

**`__DEV__`**: We previously used an inline `<script>` tag to set the `__DEV__` mode flag. I tried using the "initialization" tags for this, but they fire too late. I moved it to `<html data-developer-mode="1">`, which seems OK everywhere.

**CSP Scope**: Only the CSP header on the original request appears to matter -- you can't refine the scope by emitting headers on CSS/JS. To reduce confusion, I disabled the headers on those response types. More headers could be disabled, although we're likely already deep in the land of diminishing returns.

**Initialization**: The initialization sequence has changed slightly. Previously, we waited for the <script> in bottom of the document to evaluate. Now, we go fishing for tags when domcontentready fires.

Test Plan:
  - Browsed around in Firefox, Safari and Chrome looking for console warnings. Interacted with various Javascript behaviors. Enabled Quicksand.
  - Disabled all the framebusting, launched a clickjacking attack, verified that each layer of protection is individually effective.
  - Verified that the XHProf iframe in Darkconsole and the PHPAST frame layout work properly.
  - Enabled notifications, verified no complaints about connecting to Aphlict.
  - Hit `__DEV__` mode warnings based on the new data attribute.
  - Tried to do sketchy stuff with `data:` URIs and SVGs. This works but doesn't seem to be able to do anything dangerous.
  - Went through the Stripe and Recaptcha workflows.
  - Dumped and examined the CSP headers with `curl`, etc.
  - Added a raw <script> tag to a page (as though I'd found an XSS attack), verified it was no longer executed.

3f53718d10 Modularize rate/connection limits in Phabricator
Summary:
Depends on D18702. Ref T13008. This replaces the old hard-coded single rate limit with multiple flexible limits, and defines two types of limits:

  - Rate: reject requests if a client has completed too many requests recently.
  - Connection: reject requests if a client has too many more connections than disconnections recently.

The connection limit adds +1 to the score for each connection, then adds -1 for each disconnection. So the overall number is how many open connections they have, at least approximately.

Supporting multiple limits will let us do limiting by Hostname and by remote address (e.g., a specific IP can't exceed a low limit, and all requests to a hostname can't exceed a higher limit).

Configuring the new limits looks something like this:

  - Made a lot of requests, got cut off by the rate limit.
  - Used `curl --limit-rate -F 'data=@the_letter_m.txt' ...` to upload files really slowly. Got cut off by the connection limit. With `enable_post_data_reading` off, this correctly killed the connections //before// the uploads finished.
  - I'll send this stuff to `secure` before production to give it more of a chance.

9777c66576 Allow Phabricator to run with "enable_post_data_reading" disabled
Summary:
Ref T13008. Depends on D18701. The overall goal here is to make turning `enable_post_data_reading` off not break things, so we can run rate limiting checks before we read file uploads.

The biggest blocker for this is that turning it off stops `$_FILES` from coming into existence.

This //appears// to mostly work. Specifically:

  - Skip the `max_post_size` check when POST is off, since it's meaningless.
  - Don't read or scrub $_POST at startup when POST is off.
  - When we rebuild REQUEST and POST before processing requests, do multipart parsing if we need to and rebuild FILES.
  - Skip the `is_uploaded_file()` check if we built FILES ourselves.

This probably breaks a couple of small things, like maybe `__profile__` and other DarkConsole triggers over POST, and probably some other weird stuff. The parsers may also need more work than they've received so far.

I also need to verify that this actually works (i.e., lets us run code without reading the request body) but I'll include that in the change where I update the actual rate limiting.

Test Plan:
  - Disabled `enable_post_data_reading`.
  - Uploaded a file with a vanilla upload form (project profile image).
  - Uploaded a file with drag and drop.
  - Used DarkConsole.
  - Submitted comments.
  - Created a task.
  - Browsed around.

c71d9c601f Pass all Throwables to Exception Handlers, not just Exceptions
Summary:
Ref T12855. PHP7 introduced "Throwables", which are sort of like super exceptions. Some errors that PHP raises at runtime have become Throwables instead of old-school errors now.

The major effect this has is blank pages during development under PHP7 for certain classes of errors: they skip all the nice "show a pretty error" handlers and

This isn't a compelete fix, but catches the most common classes of unexpected Throwable and sends them through the normal machinery. Principally, it shows a nice stack trace again instead of a blank page for a larger class of typos and minor mistakes.

a1f5d37357 Improve the behavior of PhabricatorFileEditField for Macros
Summary:
See D17848. This improves things a little bit in two cases:

Case 1:

  - Create a macro.
  - Pick a valid file.
  - Pick an invalid name.
  - Submit form.
  - Before patch: your file is lost and you have to pick it again.
  - After patch: your file is "held" in the form, you just can't see it in the UI. If you submit again, it keeps the same file. If you pick a new file, it uses that one instead.

Case 2:

  - Apply D17848.
  - Delete the `if ($value) {` thing that I'm weirded out about (see inline).
  - Edit a macro.
  - Don't pick a new file.
  - Before patch: error, can't null the image PHID.
  - Afer patch: not picking a new file means "keep the same file", but you can't tell from the UI.

Basically, the behaviors are good now, they just aren't very clear from the UI since "the field has an existing/just-submitted value" and "the field is empty" look the same. I think this is still a net win and we can fix up the UI later.

Test Plan: See workflows above.

3698e4a14f Update rate limiting for APCu and X-Forwarded-For
Summary:
Ref T12612. This updates the rate limiting code to:

  - Support a customizable token, like the client's X-Forwarded-For address, rather than always using `REMOTE_ADDR`.
  - Support APCu.
  - Report a little more rate limiting information.
  - Not reference nonexistent documentation (removed in D16403).

I'm planning to put this into production on `secure` for now and then we can deploy it more broadly if things work well.

Test Plan:
 - Enabled it locally, used `ab -n 100` to hit the limit, saw the limit enforced.
 - Waited a while, was allowed to browse again.

b479941ceb Make "Range" HTTP header work for Celerity static resource requests
Summary:
Ref T7567. In T8266 I fixed a bunch of obscure "Range" issues, but only for file downloads -- not for Celerity.

Extend all that stuff to Celerity, which is fortunately much easier.

I believe this will fix Conpherence sounds in Safari.

Test Plan:
  - Wrote out an HTTP request in a text file with `Range: bytes=0-1` and similar, piped it to localhost with `cat request.txt | nc localhost 80`, saw server return appropriate range responses consistent with file behavior after T8266, which all seems to work.
  - Also did that for files to try to make sure I wasn't breaking anything.

c4c3de7bfa Correct an issue where the wrong "Content-Length" was set for partial content responses
Summary: Ref T8266. Although we compute this correctly above, we ignored it when actually setting the header. Use the computed value to set the "Content-Length" header. This is consistent with the spec/documentation.

Test Plan: Before, some audio (like `rain.mp3`) was pretty spotty about loading in Safari. It now loads consistently for me locally.

c07ec8fee6 Preserve nonstandard ports during 404 redirects which add "/" to the ends of URIs
Summary:
Fixes T12058. When the user visits `/maniphest`, for example, we redirect to `/maniphest/`.

Since this redirect is very low-level (at the Aphront level, below the Site level) we need to preserve the request Host rather than correct it to `PhabricatorEnv::getURI()` or similar -- the request may be hiting a different Site like a blog domain.

Currently, we do not preserve the port. Instead, preserve the port if it is not a standard port for the protocol (80 for http, 443 for https).

Test Plan:
  - Made a request with a missing slash and a normal port in my browser, got redirected normally.
  - Made a request with a missing slash and a nonstandard port, got redirected on the same port.

5215eb3067 Fix an issue where unexpected debugging output would run afoul of automatic compression
Summary:
If you put "echo" or "print" statements into the code at random places (as I frequently do during development), they would emit before we enabled compression.

This would confuse the compression mechanism and browser. I tried using `headers_sent()` to selectively disable compression but that didn't appear to fix this interaction (I think emitting this text does not cause headers to send, but does let contet escape into some buffer which the compressor can not access).

Instead, push the header down a little bit so it renders after we activate compression.

Also make it slightly fancier / more hideous. WOW.

ffdc082852 Add a wide range of HTTP-request-based setup checks
Summary:
Ref T11553. With some regularity, users make various configuration mistakes which we can detect by making a request to ourselves.

I use a magical header to make this request because we want to test everything else (parameters, path).

  - Fixes T4854, probably. Tries to detect mod_pagespeed by looking for a header. This is a documentation-based "fix", I didn't actually install mod_pagespeed or formally test this.
  - Fixes T6866. We now test for parameters (e.g., user somehow lost "QSA").
  - Ref T6709. We now test that stuff is decoded exactly once (e.g., user somehow lost "B").
  - Fixes T4921. We now test that Authorization survives the request.
  - Fixes T2226. Adds a setup check to determine whether gzip is enabled on the web server, and attempts to enable it at the PHP level.
  - Fixes `<space space newline newline space><?php` in `preamble.php`.

Test Plan: Tested all of these setup warnings, although mostly by faking them.

6058d3305f Normalize remote IP addresses when writing to logs, etc
Summary:
Ref T11939. IPv4 addresses can normally only be written in one way, but IPv6 addresses have several formats.

For example, the addresses "FFF::", "FfF::", "fff::", "0ffF::", "0fFf:0::", and "0FfF:0:0:0:0:0:0:0" are all the same address.

Normalize all addresses before writing them to logs, etc, so we store the most-preferred form ("fff::", above).

Test Plan:
Ran an SSH clone over IPv6:

It worked; verified that address read out of `SSH_CLIENT` sensibly.

Faked my remote address as a non-preferred-form IPv6 address using `preamble.php`.

9730f5a34f Allow custom Sites to have custom 404 controllers
Summary:
Currently, custom Sites must match `.*` or similar to handle 404's, since the fallback is always generic.

This locks them out of the "redirect to canonicalize to `path/` code", so they currently have a choice between a custom 404 page or automatic correction of `/`.

Instead, allow the 404 controller to be constructed explicitly. Sites can now customize 404 by implementing this method and not matching everything.

(Sites can still match everything with a catchall rule if they don't want this behavior for some reason, so this should be strictly more powerful than the old behavior.)

See next diff for CORGI.

Test Plan:
  - Visited real 404 (like "/asdfafewfq"), missing-slash-404 (like "/maniphest") and real page (like "/maniphest/") URIs on blog, main, and CORGI sites.
  - Got 404 behavior, redirects, and real pages, respectively.

78040e0ff5 Run "DatabaseSetup" checks against all configured hosts
Summary:
Ref T10759. Currently, these checks run only against configured masters. Instead, check every host.

These checks also sort of cheat through restart during a recovery, when some hosts will be unreachable: they test for "disaster" by seeing if no masters are reachable, and just skip all the checks in that case.

This is bad for at least two reasons:

  - After recent changes, it is possible that //some// masters are dead but it's still OK to start. For example, "slowvote" may have no master, but everything else is reachable. We can safely run without slowvote.
  - It's possible to start during a disaster and miss important setup checks completely, since we skip them, get a clean bill of health, and never re-test them.

Instead:

  - Test each host individually.
  - Fundamental problems (lack of InnoDB, bad schema) are fatal on any host.
  - If we can't connect, raise it as a //warning// to make sure we check it later. If you start during a disaster, we still want to make sure that schemata are up to date if you later recover a host.

In particular, I'm going to add these checks soon:

  - Fatal if a "master" is replicating.
  - Fatal if a "replica" is not replicating.
  - Fatal if a database partition config differs from web partition config.
  - When we let a database off with a warning because it's down, and later upgrade it to a fatal because we discover it is broken after it comes up again, fatal everything. Currently, we keep running if we "discover" the presence of new fatals after surviving setup checks for the first time.

Test Plan:
  - Configured with multiple masters, intentionally broke one (simulating a disaster where one master is lost), saw Phabricator still startup.
  - Tested individual setup checks by intentionally breaking them.
