821df3364e Hide the "hidden" fields on custom form previews
Summary:
Hide the "hidden" fields on custom form previews.

Before this change, Phabricator's custom form preview doesn't actually hide the
hidden fields, instead it shows them at 50% opacity. It looks cluttered and
doesn't provide a very useful "preview" at all.

This just hides some fields via CSS.

c9ff6ce390 Add CSRF to SMS challenges, and pave the way for more MFA types (including Duo)
Summary:
Depends on D20026. Ref T13222. Ref T13231. The primary change here is that we'll no longer send you an SMS if you hit an MFA gate without CSRF tokens.

Then there's a lot of support for genralizing into Duo (and other push factors, potentially), I'll annotate things inline.

Test Plan: Implemented Duo, elsewhere.

1729e7b467 Improve UI for "wait" and "answered" MFA challenges
Summary:
Depends on D19906. Ref T13222. This isn't going to win any design awards, but make the "wait" and "answered" elements a little more clear.

Ideally, the icon parts could be animated Google Authenticator-style timers (but I think we'd need to draw them in a `<canvas />` unless there's some clever trick that I don't know) or maybe we could just have the background be like a "water level" that empties out. Not sure I'm going to actually write the JS for either of those, but the UI at least looks a little more intentional.

550028a882 Allow Phriction document edits to be saved as drafts
Summary:
Depends on D19661. Ref T13077. See PHI840.

When a user edits a page normally, add a "Save as Draft" button. Much of this change is around making that button render and behave properly: it needs to be an `<input type="submit" ...>` so browsers submit it and we can figure out which button the user clicked.

Then there are a few minor rules:

  - If you're editing a page which is already a draft, we only give you "Save as Draft". This makes edits to update/revise a draft more natural.
  - Highlight "Publish" if it's a likely action that you might want to take.

Internally, there are two types of edits. Both types create a new version with the new content. However:

  - A "content" edit sets the version shown on the live page to the newly-created version.
  - A "draft" edit does not update the version shown on the live page.

Test Plan: Edited a published document, edited the draft. Published documents. Reverted documents.

8a6d767843 Fix a minor text alignment issue for static text comment actions like "Accept Revision"
Summary:
Ref T13187. See PHI836. The "action" comment actions in Differential (Accept, Reject, etc) render a single line of descriptive text. This is currently slightly misaligned.

Give it similar sizing information to the label element to the left, so it lines up properly.

fab37aa4e3 When accepting revisions, allow users to accept on behalf of a subset of reviewers
Summary:
Ref T12271. Currenty, when you "Accept" a revision, you always accept it for all reviewers you have authority over.

There are some situations where communication can be more clear if users can accept as only themselves, or for only some packages, etc. T12271 discusses some of these use cases in more depth.

Instead of making "Accept" a blanket action, default it to doing what it does now but let the user uncheck reviewers.

In cases where project/package reviewers aren't in use, this doesn't change anything.

For now, "reject" still acts the old way (reject everything). We could make that use checkboxes too, but I'm not sure there's as much of a use case for it, and I generally want users who are blocking stuff to have more direct accountability in a product sense.

Test Plan:
  - Accepted normally.
  - Accepted a subset.
  - Tried to accept none.
  - Tried to accept bogus reviewers.
  - Accepted with myself not a reviewer
  - Accepted with only one reviewer (just got normal "this will be accepted" text).

bb633fb42a Clean up the Diffusion search UI a little bit
Summary:
Ref T156. @vlada recently implemented filename search in Diffusion, this cleans up the UI a little bit:

  - Instead of showing one search box with two different buttons, let the submit buttons appear to the right of the text boxes and separate the search modes.
  - Clean up the results a little bit (don't show columns which don't exist).

b57b72368c Move "Remote URI" to modern repository editor thing
Summary:
Ref T2231. Allows you to edit the remote URI and credentials.

This is a little bit funky because I'm reusing some of the pages on the new (not-yet-hooked-up) create form. Specifically, it had pages like this:

  - Repo Type
  - Name/Callsign/Remote
  - Auth
  - Done

I split "Name/Callsign/Remote" into "Name/Callsign" and "Remote", then when editing the remote I just take you through "Remote" and "Auth" and then back. This lets us reuse the giant pile of protocol/URI sanity checking logic and ends up being pretty clean, although it's a little weird that the "Create" controller does both full-create and edit-remote.

Test Plan: See screenshots.
