Ref T1191. Now that the whole database is covered, we don't need to do as much work to build expected schemata. Doing them database-by-database was helpful in converting, but is just reudndant work now.
Instead of requiring every application to build its Lisk objects, just build all Lisk objects.
I removed `harbormaster.lisk_counter` because it is unused.
It would be nice to autogenerate edge schemata, too, but that's a little trickier.
Test Plan: Database setup issues are all green.

Couple of tweaks:
- If a conpherence has no participants, we fail to `attachParticipants()`. This can happen if you leave a Conpherence as the last participant, then visit the URI again explicitly.
- If you can't load any transactions (usually, because you don't have permission to view a thread's transactions), we try to attach `null` instead of `array()`. This can happen if you attempt to view a thread you don't have permission to see. A more general fix would be to tweak the load/filtering order, but I'm leaving that for another time since it's more involved and only gives us a small performance gain in unusual sitautions.
- `initializeNewThread()` should be declared `static`.
Test Plan:
- Viewed a thread with no participants, got proper policy error.
- Viewed a thread I couldn't see, got proper policy error.
- Grepped for `initializeNewThread()`.

- With strict MySQL settings, we try to insert `null` into the non-nullable `messageCount` field. Add an `initializeNew...` method.
- If we don't create a new conpherence (for example, because the message body is empty), we fatal on `getPHID()` right now.
Also, make this stuff a little easier to test.
Test Plan: Used `mail_handler.php` to receive empty conpherence mail, and new-thread conpherence mail.

Also remove some duplicate `$id` and `$phid` properties. These are declared on `PhabricatorLiskDAO` and do not need to be redeclared.
Test Plan: Ran the `testEverythingImplemented` unit test, which verifies that all classes conform to the interface.

Summary: somewhere along the line this broke. Before this patch we fail the visibility check since its based on Conpherence Participants which don't get created and attached until applyExternalEffects. Believe it or not, this was the least gross fix I could come up with; since the permission check is done SO early most other ideas I had involved creating a dummy participant object to pass the check then handling things for real later on... Ref T3723.

Summary:
removes the whole custom image thing, instead using a more standard application crumbs. Gives this glorious space back to the compose area which is now tens of pixels taller. Also defaults it to the people widget. Basically, fixes T3160.
For now, you **CAN NOT** edit the title of a conpherence. I didn't want to jam in too much here. Next diff will be to change the widget icons into the dropdown switcher, which will also bring back the editing of titles.
Test Plan: looked at conpherence and it was pretty. Resized it vigorously and it wasn't too bad.

Summary:
the JS is fragile with respect to the tokenizer coming in from the people widget. make sure to always try to load this up. Note this generally needs to get cleaned up where the server should only send down the *exact* bits the client needs. This is all TODO as part of getting this on mobile perfectly. Also note this fragility exists still in that you can break conpherence by clicking quickly before the initial tokenizer load loads.
For old bad data, at some point we weren't updating participation as well as we do today in the editor class. the result is with the migration and code change some conversation participants have bad "last seen message" counts. the simplest case is the test user talking to themselves -- threads before the editor code fixes / changes will have the entire thread as unread for these folks. The other buggy case I saw was where the "last reply" to a thread wasn't being count. These issues showed up for threads February and older which is old.
Test Plan: edited conpherence meta data and no JS bugs. pontificated and no JS bugs. added a person and no JS bugs.

I realized that we don't need to keep track of cacheTitle and cachePhoto since those are based off recent participation handles and dynamic relative to who is viewing it. Also kept the "last seen phid" as I think that will be useful to have auto-scroll to where you last read. Ref T2867.

started a thread with a test user, thus two participants total. Replied to conpherence, toggling notification settings in between. Verified 1 or 2 emails were sent as appropos to the current toggle.

Summary: files widget updates as new files are added. made basically all edits "ajax" except for when you change the conpherence image which just does a reload. I will fix this in a future diff but it was starting to spiral out of control a bit.
Test Plan: commented on a task with files and noted the updated file widget. updated header image via drag and drop and dialogue -- noted a full reload. cropped image and re-titled conpherence - ajax updated as expected.

Summary: pre-patch, when you upload a photo if the conphernece has a name it gets cleared. Post patch this no longer happens. Patch also makes the case where you delete the conpherence name have more sensical text.
Test Plan: named a conpherence, uploaded an image, verified the name stayed the same. Deleted a conpherence name by changing the text to nothing and verified it work correctly, including having good transaction text.
