Ref T7803. Ref T5873. I want to drive Conduit through more shared infrastructure, but can't currently add parameters automatically.
Put a `getX()` around the `defineX()` methods so the parent can provide default behaviors.
Also like 60% of methods don't define any special error types; don't require them to implement this method. I want to move away from this in general.
Test Plan:
- Ran `arc unit --everything`.
- Called `conduit.query`.
- Browsed Conduit UI.

Summary:
Ref T6013. I accidentally made this cost explosviely huge when fixing macros for logged out users in D10411.
Specifically, we'd load all the macros, which would load all the files, which would load all the macros (to do policy checks), which would fill out of cache I think (but maybe only some of the time?). Anyway, bad news.
Instead, only load the files if we need them.
Test Plan: Viewed macro main page, macro detail, used a macro, used a meme, edited a macro, edited audio.

Summary: Fixes T5884. Macro images are no longer public on most installs. We could generate tokens for them, but this (using Conduit to pull the file data) is easier and more correct.
Test Plan: Logged a bot into IRC and had it spam part of a macro before being killed for flooding.

Summary: Ran `arc lint --apply-patches --everything` over rP, mainly to change double quotes to single quotes where appropriate. These changes also validate that the `ArcanistXHPASTLinter::LINT_DOUBLE_QUOTE` rule is working as expected.

Fixes T2820
Grepped for `PhabricatorFileImageMacro`, a common approach to load image macros from storage. Cleaned up file loading too (in most cases where I could be sure that I won't break anything).
Did not touch the `add_macro.php` util script, since many users will assume the user `ubuntu` or `ec2-user` to run the script. Add no sessions and no CSRF protection measures...
Test Plan:
Browsed around all kinds of places. Created and looked at memes, created and edited macros. Used them in Remarkup (with flushed cache). Used `macro.query`, verified it did not crash (that's always a good sign).
Could not verify object handles, since I have no idea where they appear right now.

Summary: Fixes T2698. When applications are installed, their Conduit calls should drop out. This will also let us land Releeph without exposing Conduit calls.
Test Plan:
- Viewed Conduit console; uninstalled some applications and verified their calls dropped out.
- Tried to make an uninstalled call; got an appropriate error.
