Summary: Ref T4324. Ref T5284. This adds server-side support for keeping track of a set of PHIDs that the Aphlict clients have subscribed to. Instead of broadcasting a notification to all clients (after which the clients can poll `/notification/individual` in order to determine whether or not they are interested in the notification), transmit notifications only to clients that have subscribed to a PHID that is relevant to the notification.

Ref T4843. This adds support to `javelin_tag()` for an `aural` attribute. When specified, `true` values mean "this content is aural-only", while `false` values mean "this content is not aural".
- I've attempted to find the best modern approaches for marking this content, but the `aural` attribute should let us change the mechanism later.
- Make the "beta" markers on application navigation visual only (see T4843). This information is of very low importance, the application navigation is accessed frequently, and the information is available on the application list.
- Partially convert the main navigation. This is mostly to test things, since I want to get more concrete feedback about approaches here.
- Add a `?__aural__=1` attribute, which renders the page with aural-only elements visible and visual-only elements colored.

Summary:
Ref T4398. This is roughly a "sudo" mode, like GitHub has for accessing SSH keys, or Facebook has for managing credit cards. GitHub actually calls theirs "sudo" mode, but I think that's too technical for big parts of our audience. I've gone with "high security mode".
This doesn't actually get exposed in the UI yet (and we don't have any meaningful auth factors to prompt the user for) but the workflow works overall. I'll go through it in a comment, since I need to arrange some screenshots.

Summary:
Fixes T4365. See discussion in D8123.
This implements the most conservative solution of approaches discussed in D8123. Basically:
- When you search in primary search, we overwrite "query" in your default (topmost) search filter, and execute that.
This doesn't implement any of the other "sticky" stuff, where the query sticks around. Maybe we'll do that eventually, but it gets messy and could be confusing. Practically, this addresses the major use case in the wild, which is to make the menu bar search mean "Open Tasks" by default.
This also removes the old, obsolete "search scope" stuff. A long time ago, searching from within Maniphest would search tasks, etc., but this was pretty weird and confusing and is no longer used, and no one complained when we got rid of it.
Test Plan: Dragged "Open Tasks" to my top search, searched for "asdf", got "asdf in open tasks" results.

Ref T4339. Ref T4310. Currently, sessions look like `"afad85d675fda87a4fadd54"`, and are only issued for logged-in users. To support logged-out CSRF and (eventually) external user sessions, I made two small changes:
- First, sessions now have a "kind", which is indicated by a prefix, like `"A/ab987asdcas7dca"`. This mostly allows us to issue session queries more efficiently: we don't have to issue a query at all for anonymous sessions, and can join the correct table for user and external sessions and save a query. Generally, this gives us more debugging information and more opportunity to recover from issues in a user-friendly way, as with the "invalid session" error in this diff.
- Secondly, if you load a page and don't have a session, we give you an anonymous session. This is just a secret with no special significance.
This does not implement CSRF yet, but gives us a client secret we can use to implement it.
Test Plan:

Ref T3684. The URI itself is reflected in a few places. It is generally not dangerous because we only let you add random stuff to the end of it for one or two controllers (e.g., the file download controller lets you add "/whatever.jpg"), but:
- Remove it entirely in the main request, since it serves no purpose.
- Remove query parameters in Ajax requests. These are available in DarkConsole proper.
Also mask a few things in the "Request" tab; I've never used these fields when debugging or during support, and they leak quasi-sensitive information that could get screenshotted or over-the-shoulder'd.
I didn't mitgate `__metablock__` because I think the threat is so close to 0 that it's not worthwhile.

This creates a common form look and feel across the site. I spent a bit of time working out a number of kinks in our various renderings. Some things:
- Font Styles are correctly applied for form elements now.
- Everything lines up!
- Selects are larger, easier to read, interact.
- Inputs have been squared.
- Consistant CSS applied glow (try it!)
- Improved Mobile Responsiveness
- CSS applied to all form elements, not just Aphront
- Many other minor tweaks.
I tried to hit as many high profile forms as possible in an effort to increase consistency. Stopped for now and will follow up after this lands. I know Evan is not a super fan of the glow, but after working with it for a week, it's way cleaner and responsive than the OS controls. Give it a try.
Test Plan: Tested many applications, forms, mobile and tablet.

Currently, `isInstalled()` and `getAllInstalledApplications()` are inconsistent:
- `isInstalled()` returns true for beta apps, even if `phabricator.show-beta-applications` is false.
- `getAllInstalledApplications()` omits beta apps if `phabricator.show-beta-applications` is false.
Making the beta config control installs (not just homepage visibility) makes far more sense as we roll out more thorough application integrations.
Make `isInstalled()` respect beta, and clean up some callsites.
D5602 builds on this.
Test Plan: Installed/uninstalled beta apps, verified Conpherence menu/panel and other application integrations dropped out of the UI.
