Summary:
This hasn't been updated in a bit more than a year (last updated in D18594) and we've accumulated a fair number of SQL patches. Update it.
This is mostly automatic (with `bin/storage quickstart`), except:
- Manual edit to one migration for a missed callsite to `DashboardInstall`.
- Replaced two InnoDB tables that still have FULLTEXT indexes with MyISAM (see rP6cedd4a95cfc).
This is not really possible to review and more for reference than examination. `bin/storage quickstart` has historically worked correctly.
Test Plan: I have great faith that `bin/storage quickstart` is a script which creates a big `.sql` file.

Summary:
The quickstart SQL hasn't been regenrated in a while, mildly impacting unit test and instance startup times.

Test Plan:
- Saw database setup drop from ~10,500ms to ~7,500ms locally.
- Visually inspected diff, changes looked expected.

Summary:
Fixes T7422. After the recent fix for "sort" columns, we can end up with invalid SQL in some cases when running quickstart.
In particular, we do "COLLATE binary CHARACTER SET utf8_general_ci" (which is invalid).
Preprocess these so we get "COLLATE utf8 CHARACTER SET utf8_general_ci" (which is valid and correct).
Test Plan: Ran `bin/storage upgrade -f --namespace blahblhbaba` with and without `--disable-utf8mb4`.

Summary:
One advantage I wanted to get out of T1191 is automated rebuilds of `quickstart.sql`. If they don't actually work, I'd like to know sooner rather than later. We haven't rebuilt in a couple months, so give it a shot.
Ran into two issues:
- Some very old patches specify overlong keys which don't work if your default charsets are utf8mb4. Shorten these. No real users have applied these in a very long time.
- Some gymnastics around `corpus` for the new Conpherence search index.
Test Plan:
- Ran `arc unit --everything`, got clean results.
- Cost to do a storage upgrade on an empty namespace dropped from ~4s to ~3s.

This puts new installs into utf8mb4 mode immediately without requiring storage adjustment.

Summary:
Splits out the SQL changes. These are most of the changes, but primarily mechanical:
- Moved "initialize.sql" to "0000.legacy.sql" and partially reverted to an older version, such that patches 0000 + 000 + 001 + ... + 137 put us in the right state when applied sequentially.
- Removed "create database" commands from all SQL. These are handled by separate DB patches now, so we have the data to do operations like "storage databases" (list databases) and "storage destroy" (drop databases).
- Removed "phabricator_" namespace from all SQL, and replaced with "{$NAMESPACE}_" token so we can namespace databases.
- Shortened some column lengths so patches apply correctly if originally created as InnoDB; also a few similar tweaks elsewhere.
Test Plan: See D2323 for discussion and test plan.
