Summary: Ref T7153. I am not sure if this is 100% correct because sometimes you have to POST vs GET and I don't know if the redirect response will / can do the right thing? I think options to fix this would be to 1) restrict this functionality to JUST the Phabricator OAuth provider type or 2) something really fancy with an HTTP(S) future. The other rub right now is when you logout you get half auto-logged in again... Thoughts on that?
Test Plan: setup my local instance to JUST have phabricator oauth available to login. was presented with the dialog automagically...!

Summary:
Ref T4398. This prompts users for multi-factor auth on login.
Roughly, this introduces the idea of "partial" sessions, which we haven't finished constructing yet. In practice, this means the session has made it through primary auth but not through multi-factor auth. Add a workflow for bringing a partial session up to a full one.
Test Plan:
- Used Conduit.
- Logged in as multi-factor user.
- Logged in as no-factor user.
- Tried to do non-login-things with a partial session.
- Reviewed account activity logs.

Summary: Ref T4310. Ref T3720. Session operations are currently part of PhabricatorUser. This is more tightly coupled than needbe, and makes it difficult to establish login sessions for non-users. Move all the session management code to a `SessionEngine`.

- Regenerated Conduit certificate.
- Verified Conduit sessions were destroyed.
- Logged out.
- Logged in.
- Ran conduit commands.

Summary:
Ref T4310. This is a small step toward separating out the session code so we can establish sessions for `ExternalAccount` and not just `User`.
Also fix an issue with strict MySQL and un-admin / un-disable from web UI.
Test Plan: Logged in, logged out, admined/de-admin'd user, added email address, checked user log for all those events.

Summary:
This commit doesn't change license of any file. It just makes the license implicit (inherited from LICENSE file in the root directory).
We are removing the headers for these reasons:
- It wastes space in editors, less code is visible in editor upon opening a file.
- It brings noise to diff of the first change of any file every year.
- It confuses Git file copy detection when creating small files.
- We don't have an explicit license header in other files (JS, CSS, images, documentation).
- Using license header in every file is not obligatory: http://www.apache.org/dev/apply-license.html#new.
This change is approved by Alma Chao (Lead Open Source and IP Counsel at Facebook).
Test Plan: Verified that the license survived only in LICENSE file and that it didn't modify externals.

Summary: Currently, the logout link is this awkward form in the footer since we have to CSRF it. Enable a GET + dialog + confirm workflow instead so the logout link can just be a link instead of this weird mess.
