Summary:
Rebuilds the UI in Differential commenting. Specifically we look at the following design patterns:
**To the author:**
- The author of the diff should be able to easily identify what comments are done and not done.
- We keep undone comments yellow
- Clicking done turns comment block into 'unsubmitted state'
**To the reviewer:**
- Easier understanding of unsubmitted states
- All conversations to be yellow/important
**Todo**
- Not all color CSS states correct
- Unpulished checkbox support

Test leaving comments, published and unpublished. Checking Done, unpublished and published. Check delete states.

Summary: Fixes T7655. We'll set tighter spacing around edit clusters. Also darkened up the date marker and remove unused `phabricator-transaction-view` CSS that was still scattered around the site.

Summary:
Currently, code blocks inside Phabricator remarkup are constrained to ~80col. There's no technical or design reason for this -- it looks and works fine without the restriction. Join the new world of fluid widths.
(Note that //inline// comments can get a bit whacky in some cases with large code blocks, but we have no restrictions there currently, so this doesn't break anything.)

all sorts of stuff
- made comment form width flexible
- made margins element specific rather than part of differential-primary-pane
- made box elements all veritically align left and right until code stuff
- re-factored width calculaton stuff a bunch so only the code section has to suffer from max-width calculations; everything else can flex
- made colspan 3 for rightmost table header element. this is so the "View Options" UI element ends up lining up correctly with the "Show All Lines" element just below
Test Plan: looked at revision view and changeset view and it all looked hot. note I did not test what things looked like with different word wrap values; that should still work given the re-factoring and not re-design here. also toggled haunted panel mode and it looked good.

Summary:
- Adds "Commandeer Revision", to allow you to plunder revisions from those lost to sea (e.g., interns who have left or co-workers who are dealing with a family emergency).
- Removes admin-abandon to simplify things, since you can just Commandeer + Abandon now.
- There are other workarounds available but this is the natural/expected workflow (and the one everyone always asks for) and there's no real reason not to allow it.

- In "haunted" mode, "z" now toggles through three modes: normal, comment area only, and comment + previews.

I want to add comments to commits, and they should obviously share code with the
nearly-identical comments in Maniphest and Differential. Unify code/style as
much as possible.

- Looked at a bunch of different Differential and Maniphest comments; they
appeared to render identically to how they looked before.
- Tested some edge cases like anchors and "show details" on description edits
in Maniphest.

Summary:
This diff restructures the DOM and alters some CSS within differential.
Original goal was to unify these codepaths more fully into a base class or
classes, but they have quite a bit of custom code such that didn't feel too
compelling in practice. It also felt related to feed stories as I thought
about the more general version(s) of this code...
Also deleted some CSS from maniphest that wasn't doing anything.
Test Plan:
looked at a differential diff and liked what I saw. spent a bunch
of time trying out different types of comments and etc.

Summary:
Allows you to link to comments with "D123#3" or "T123#3", then adds a pile of JS
to try to make it not terrible. :/
The thing I'm trying to avoid here is when someone says "look at this!
http://blog.com/#comment-239291" and you click and your browser jumps somewhere
random and you have no idea which comment they meant. Since I really hate this,
I've tried to avoid it by making sure the comment is always highlighted.
Test Plan:
Put T1#1 and D1#1 in remarkup and verified they linked properly.
Clicked anchors on individual comments.
Faked all comments hidden in Differential and verified they expanded on anchor
or anchor change.
