Summary: Depends on D19810. Ref T13217. Ref T13216. I mostly used `grep implode | grep OR` and `grep implode | grep AND` to find these -- not totally exhaustive but should be a big chunk of the callsites that are missing `%LO` / `%LA`.
Test Plan:
These are tricky to test exhaustively, but I made an attempt to hit most of them:
- Browsed Almanac interfaces.
- Created/browsed Calendar events.
- Enabled/disabled/showed the lock log.
- Browsed repositories.
- Loaded Facts UI.
- Poked at Multimeter.
- Used typeahead for users and projects.
- Browsed Phriction.
- Ran various fulltext searches.
Not sure these are reachable:
- All the lint stuff might be dead/unreachable/nonfunctional?

Summary:
Fixes T10423. Ref T11524. This changes `diffusion.rawdiffquery` to return a file PHID instead of a blob of data.
This is better in general, but particularly better for huge diffs (as in T10423) and diffs with non-utf8 data (as in T10423).
Test Plan:
- Used `bin/differential extract` to extract a latin1 diff, got a clean diff.
- Used `bin/repository reparse --herald` to rerun herald on a latin1 diff, got a clean result.
- Pushed latin1 diffs to test commit hooks.
- Triggered the the too large / too slow logic.
- Viewed latin1 diffs in Diffusion.
- Used "blame past this change" in Diffusion to hit the `before` logic.

Summary:
Ref T4705 (there are also some other adjacent related tasks dealing with URIs).
Currently, we issue a "get repositories matching URIs: ..." query by loading every possible repository and then checking their URIs in PHP.
Instead, put URIs in a separate table. I plan for each repository to potentially have multiple URIs soon, so this prepares for that.
Test Plan:
- Ran migrations.
- Looked at index table, made sure it appeared sensible.
- Ran some queries by `uri` to find repositories, found the repositories I expected.
- Updated the remote URI of a repository, saw queries / index update appropriately.

Ref T2450. Ref T9319. This is still a bit messy, but not quite so bad as it was: instead of using a single call to get both blame information and file content, use `diffusion.blame` for blame information.
This will make optimizations to both blame and file content easier.
Test Plan: Viewed a bunch of blame (color on/off, blame on/off).

Summary:
Ref T2783. I think this served two purposes:
- Improving performance in cases where we "know" a repository is local.
- Preventing loops.
It is now obsolete:
- After D11476, refs can almost always resolve on a fast path.
- As T2783 moves forward, we can usually no longer know when a repository is local without actually looking it up -- almost everything is allowed to run anywhere.
- The cluster behavior in D11475 now prevents loops.
Test Plan: `grep`, browsed around. This didn't really do much of anything anymore.

Summary:
Ref T4994. This stuff works:
- You can dump a blob of coverage information into `diffusion.updatecoverage`. This wipes existing coverage information and replaces it.
- It shows up when viewing files.
- It shows up when viewing commits.
This stuff does not work:
- When viewing files, the Javascript hover interaction isn't tied in yet.
- We always show this information, even if you're behind the commit where it was generated.
- You can't do incremental updates.
- There's no aggregation at the file (this file has 90% coverage), diff (the changes in this commit are 90% covered), or directory (the code in this directory has 90% coverage) levels yet.
- This is probably not the final form of the UI, storage, or API, so you should expect occasional changes over time. I've marked the method as "Unstable" for now.
Test Plan:
- Ran `save_lint.php` to check for collateral damage; it worked fine.
- Ran `save_lint.php` on a new branch to check creation.
- Published some fake coverage information.
- Viewed an affected commit.
- Viewed an affected file.
