Summary: Fixes T9715. Adds a MySQL-based lock to ensure that schema migrations are not applied on multiple hosts simultaneously. Test Plan: Ran `./bin/storage upgrade` concurrently. One invocation was successful whilst the other hit a `PhutilLockException`.
