See PHI325. When a transaction group in Differential (or Pholio) only has an inline comment, it renders with a "V" caret but no actual dropdown menu.
This caret renders in a "disabled" color, but the color is "kinda grey". The "active" color is "kinda grey with a dab of blue". Here's what they look like today:

Summary:
Ref T10233. See PHI231. When users ignore the `arc land` prompt about bad revision states, make it explicitly clear in the transaction log that they broke the rules.
You can currently figure this out by noticing that there's no "This revision is accepted and ready to land." message, but it's unrealistic to expect non-expert users to look for the //absence// of a message to indicate something, and this state change is often relevant.

Summary: This inverts colors and icons a bit, so they're not as harsh. So instead of a dark green item with white icon, its now light green with a dark green icon. I've also changed all text and comment boxes to be "grey" visually to separate out the UI from converation/actions. Give it a spin and let me know how this feels. I still need to update the comment UI.

I removed the ajax-edit and ajax-remove interactions, becuase they were prohibitively complex to get working given that the entire menu has to change too. Instead, the page just reloads. This works perfectly fine in practice.
If we want to restore these in the future, we should have the server re-render the entire transaction group or something. I think very little is lost here, though.

Summary:
Ref T2222. This restores the "N older comments are hidden." shield to all ApplicationTransactions applications. Roughly the rule this uses is that transactions older than your most recent comment are hidden, under the assumption that you've already read and dealt with them, since you replied afterward. Then we show your last comment to remind/contextualize you, and anything afterward. We also don't hide transactions if we'd only be hiding a handfull, and we never hide the few most recent transactions.
This might need some #design help.
Test Plan:
The tricky part here is the anchor rule, which deals with the case where you follow a link to `T123#4`, but that would normally be hidden. We simulate a click on "show all" if you hit an anchor which is hidden. Here's what it looks like in Maniphest:
