Summary:
While we mostly have reasonable effective object accessibility when you lock a user out of an application, it's primarily enforced at the controller level. Users can still, e.g., load the handles of objects they can't actually see. Instead, lock the queries to the applications so that you can, e.g., never load a revision if you don't have access to Differential.
This has several parts:
- For PolicyAware queries, provide an application class name method.
- If the query specifies a class name and the user doesn't have permission to use it, fail the entire query unconditionally.
- For handles, simplify query construction and count all the PHIDs as "restricted" so we get a UI full of "restricted" instead of "unknown" handles.
Test Plan:
- Added a unit test to verify I got all the class names right.
- Browsed around, logged in/out as a normal user with public policies on and off.
- Browsed around, logged in/out as a restricted user with public policies on and off. With restrictions, saw all traces of restricted apps removed or restricted.

Summary: Ref T603. This could be a nicer UX, but limit the amount of foot-shooting that users can possibly do. You can still manage if you're really tricky ("Members of project X", then leave the project) but this should make it hard to make a mistake. It seems very unlikely any user ever intends to lock themselves out of an application.
Test Plan: Set an application's view policy to permissive ("Administrators") and nonpermissive ("No One") values. The former were accepted, the latter rejected.

Summary:
Ref T603. I had to partially revert this earlier because it accidentally blocked access to Conduit and File data for installs without "policy.allow-public", since the applications are available to "all users" but some endpoints actually need to be available even when not logged in.
This readjusts the gating in the controller to properly apply application visibility restrictions, and then adds a giant pile of unit test coverage to make sure it sticks and all the weird cases are covered.
Test Plan:
- Added and executed unit tests.
- Executed most of the tests manually, by using logged in / admin / public / disabled users.

Fixes T2691. Now, all PhabricatorActionListViews in the codebase setObjectHref to $request->getRequestURI. This value is passed over to PhabricatorActionItems right before they are rendered. If a PhabricatorActionItem is a workflow and there is no user OR the user is logged out, we used this objectURI to construct a log in URI.
Potentially added some undesirable behavior to aggressively setUser (and later setObjectURI) from within the List on Actions... This should be okay-ish unless there was a vision of actions having different user objects associated with them. I think this is a safe assumption.
Test Plan: played around with a mock all logged out (Ref T2652) and it worked!

We have a few interfaces where add "Edit", "Delete" or some other action to a list. Currently, this happens via icons, but these are cumbersome and weird, are inconsistent, can't be workflow'd, are hard to hit on desktops and virtually impossible to hit on mobile, and generally just feel iffy to me. Prominent examples are Projects and Flags. I'd like to try adding an "edit" action to Maniphest (to provide quick edit from list views, basically). It looks like some of Releeph would benefit here, as well.
Instead, provide first-class actions:

Currently, we give you an "Install" button for Beta apps even when "phabricator.show-beta-applications" is disabled. This is unclear.
Instead, disable the button and tell the user about the config when it is clicked.
Arguably, we should restructure this so you can selectively install "Beta" apps without setting the config, but almost all the beta apps are terrible so just make things consistent for now.
