Didn't implement a way to ignore the warning, which might be required - but figured this is serious and broken enough while being completely invisible 99% of the time that it's worth shouting about.
Test Plan: Messed around with $_SERVER['HTTPS'] on the server side and client_uri on the client side - saw reasonable results in all combinations.

Summary: Ref T3025. This adds a check for different client/server timezone offsets and gives users an option to fix them or ignore them.
Test Plan:
- Fiddled with timezone in Settings and System Preferences.
- Got appropriate prompts and behavior after simulating various trips to and from exotic locales.
In particular, this slightly tricky case seems to work correctly:
- Travel to NY.
- Ignore discrepancy (you're only there for a couple hours for an important meeting, and returning to SF on a later flight).
- Return to SF for a few days.
- Travel back to NY.
- You should be prompted again, since you left the timezone after you ignored the discrepancy.

Summary:
Ref T4571. There will be a very long path beyond this, but add a basic read-only mode. You can explicitly enable this to put Phabricator in a sort of "maintenance" mode today if you're swapping databases or something.
In the long term, we'll automatically degrade into this mode if the master database is down.
Test Plan:
- Enabled read-only mode.
- Browsed around.
- Didn't immediately see anything that was totally 100% broken.
Most stuff is 80-90% broken right now. For example:
- Stuff like submitting comments doesn't work, and gives you a confusing, unhelpful error.
- None of the UI really knows that it's read-only. EditEngine stuff should all hide itself and say "you can't add new comments while an install is in read-only mode", for example, but currently does not.

This swaps commenting in Maniphest over to EditEngine / stackable actions. New code doesn't have parity yet, although none of the things we're missing should technically be //strictly mandatory//. There's a list inline. I'll restore these in the next diffs.

Fixes T6559. No more flash, use Websockets. This is less aggressive than the earlier version, and retains more server logic.

In Safari, Firefox and Chrome, saw the browsers connect. Made a bunch of comments/updates and saw notifications.
Notable holes in the test plan:
- Haven't tested "wss" yet. I'll do this on secure.
- Notifications are //too fast// now, locally. I get them after I hit submit but before the page reloads.
- There are probably some other rough edges, this is a fairly big patch.

Fixes T5094. In some cases we do slightly expensive transformations to resources (inlining images, replacing URIs, building packages). We can throw cache in front of them easily since URIs are already permanently associated with a single resource.
Also browse around and move some CSS/JS into packages.

Added logging to verify the caches are working, saw moderately improved performance.

Summary:
Ref T2222. This restores the "N older comments are hidden." shield to all ApplicationTransactions applications. Roughly the rule this uses is that transactions older than your most recent comment are hidden, under the assumption that you've already read and dealt with them, since you replied afterward. Then we show your last comment to remind/contextualize you, and anything afterward. We also don't hide transactions if we'd only be hiding a handfull, and we never hide the few most recent transactions.
This might need some #design help.
Test Plan:
The tricky part here is the anchor rule, which deals with the case where you follow a link to `T123#4`, but that would normally be hidden. We simulate a click on "show all" if you hit an anchor which is hidden. Here's what it looks like in Maniphest:

- Removes the old map and changes the CelerityResourceMap API to be entirely driven by the new map.
- The new map is about 50% smaller and organized more sensibly.
- This removes the `/pkg/` URI component. All resources are now required to have unique names, so we can tell if a resource is a package or not by looking at the name.
- Removes some junky old APIs.
- Cleans up some other APIs.
- Added some feedback for `bin/celerity map`.
- `CelerityResourceMap` is still a singleton which is inextricably bound to the Phabricator map; this will change in the future.
Test Plan:
- Reloaded pages.
- Verified packaging works by looking at generated includes.
- Forced minification on and verified it worked.
- Forced no-timestamps on and verified it worked.
- Rebuilt map.
- Ran old script and verified error message.
