Ref T7149. This isn't complete and isn't active yet, but does basically work. I'll shore it up in the next few diffs.
The new workflow goes like this:
> Client, file.allocate(): I'd like to upload a file with length L, metadata M, and hash H.
Then the server returns `upload` (a boolean) and `filePHID` (a PHID). These mean:

All but the last case are uninteresting and work like exising uploads with `file.uploadhash` (which we can eventually deprecate).
In the last case:

This returns all the chunks for the file. Chunks have a start byte, an end byte, and a "complete" flag to indicate that the server already has the data.
Then, the client fills in chunks by sending them:

This stuff doesn't work yet or has some caveats:
- I haven't tested resume much.
- Files need an "isPartial()" flag for partial uploads, and the UI needs to respect it.
- The JS client needs to become chunk-aware.
- Chunk size is set crazy low to make testing easier.
- Some debugging flags that I'll remove soon-ish.
- Downloading works, but still streams the whole file into memory.
- This storage engine is disabled by default (hardcoded as a unit test engine) because it's still sketchy.
- Need some code to remove the "isParital" flag when the last chunk is uploaded.
- Maybe do checksumming on chunks.
Test Plan:
- Hacked up `arc upload` (see next diff) to be chunk-aware and uploaded a readme in 18 32-byte chunks. Then downloaded it. Got the same file back that I uploaded.
- File UI now shows some basic chunk info for chunked files:

Summary:
Fixes T5843. File storage engines use a very old "selector" mechanism which makes them difficult to extend.
This mechanism predates widespread use of `PhutilSymbolLoader` to discover available implementations at runtime. Runtime discovery has generally proven more flexible and easier to use than explicit selection (although it sometimes needs more UI to support it in cases where order or enabled/disabled flags can not be directly determined).
Use a modern runtime discovery mechanism instead of an explicit selector. This might break any installs which subclassed the `Selector`, but I believe almost no such installs exist, and they'll receive a meaningful exception upon upgrading (any custom engines will no longer implement all of the required methods).
Looking forward, this modernizes infrastructure to prepare for new "virtual" chunked-storage engines, with the eventual goal of supporting very large file uploads and data import into the Phacility cluster.
This uses D12051 to add UI to make it easier to understand the state of storage engines.

The layout CSS class already has `display: inline` set, but this is not sufficient. Browser's HTML parser doesn't care what CSS rules will be applied, it only deals with the meanings of tags.

Summary:
This allows us to CDN the cluster.
General problem is that we can't easily give each instance its own CDN URI (`giraffe.phcdn.net`) in Cloudfront, because it requires that you enumerate all aliases (and there's a limit of 100) and depends on SNI (a newish feature of SSL which allows one server to serve multiple certificates, but which doesn't have full support everywhere yet).
It's //possible// that we could eventually work around this, or use Cloudflare instead (which has a different model that seems like a slightly easier fit for CDN-domain-per-instance), but I don't want to sink a ton of work into this and want to keep things on AWS insofar as we reasonably can.
The easiest way to fix this is just to put the instance identity into URIs, then read it out when handling CDN requests. This has no effect on installs without cluster instance configuration, which is all of them except ours.
It's also slightly desirable to share this stuff, since we get to share the cache for static resources, which are always identical across instances.
So requests go from the Cloudfront gateway ("xyz.cloudfront.com") to the LB with a hard-coded instance name ("cdn.phacility.com"), which gets them routed to a balanced web machine. The web machine picks the correct instance name out of the URI, acts as that instance, and does the correct thing.
The messiest part of this is that we need "cdn.phacility.com" to be a real instance so it can serve static resources, but that's not a big deal. We have a few other hard-codes which have to be real resources for now, like we must have a merchant named "Phacility".
Test Plan:
- Viewed files with `security.alternate-file-domain` off (i.e., no file tokens).
- Viewed pages and files with `security.alternate-file-domain` on. Saw correct resource behavior, @isntance generation of URIs, and correct token redirect behavior for files.

Ref T5702. This is a forward-looking change which provides some very broad API improvements but does not implement them. In particular:
- Controllers no longer require `$request` to construct. This is mostly for T5702, directly, but simplifies things in general. Instead, we call `setRequest()` before using a controller. Only a small number of sites activate controllers, so this is less code overall, and more consistent with most constructors not having any parameters or effects.
- `$request` now offers `getURIData($key, ...)`. This is an alternate way of accessing `$data` which is currently only available on `willProcessRequest(array $data)`. Almost all controllers which implement this method do so in order to read one or two things out of the URI data. Instead, let them just read this data directly when processing the request.
- Introduce `handleRequest(AphrontRequest $request)` and deprecate (very softly) `processRequest()`. The majority of `processRequest()` calls begin `$request = $this->getRequest()`, which is avoided with the more practical signature.
- Provide `getViewer()` on `$request`, and a convenience `getViewer()` on `$controller`. This fixes `$viewer = $request->getUser();` into `$viewer = $request->getViewer();`, and converts the `$request + $viewer` two-liner into a single `$this->getViewer()`.

This was kind of a mess and I eventually removed it, during or prior to T5245. The big issue was that control flow was really hard to figure out as things went all the way down to the deepest level of infrastructure and then came back up the stack to events and transactions. The new stuff is more top-down and generally seems a lot easier and cleaner.
Consequently, actors are no longer required for edge edits. Remove the parameter.

Ref T6013. Currently, when we create a thumbnail, it gets its own (default) file visibility policy.
In particular, this causes the issue in T6013: thumbnails get "all users" visibility, which does not include logged-out users.
Instead, a thumbnail should just have the same visibility as the original file does. Enforce this:
- When loading thumbnails, reject thumbnails with invisible originals.
- When filtering thumbnails, permit thumbnails with visible originals.

In some cases, we may not share storage. Examples include:

In the particular case of files broken by the bug in T5912, we know the hash of the file's content and will only point them at a file that we can load the data for, so this fixes them.
Compaction is not hugely useful in general, but this script isn't too complex and the ability to fix damage from the bug in T5912 is desirable. We could remove this capability eventually.
Test Plan:
- Ran `files compact --all --dry-run` and sanity checked a bunch of the duplicates for actually being duplicates.
- Migrated individual files with `files compact Fnnn --trace` and verified the storage compacted and all files survived the process.
- Verified unused storage was correctly destroyed after removing the last reference to it.

Fixes T5912. When migrating files, we try to clean up the old data. However, this code isn't aware of reference counting, and unconditionally destroys the old data.
For example, if you migrate files `F1` and `F2` and they have the same data, we'll delete the shared data when we migrate `F1`. Then you'll get an error when you migrate `F2`.
Since this only affects duplicate files, it primarily hits default profile pictures, which are the most numerous duplicate files on most installs.
Test Plan:
- Verified that the theory was correct by uploading two copies of a file and migrating the first one, before applying the patch. The second one's data was nuked and it couldn't be migrated.
- Applied patch.
- Uploaded two copies of a new file, migrated the first one (no data deletion), migrated the second one (data correctly deleted).
- Uploaded two copies of another new file, `bin/remove destory'd` the first one (no data deletion), then did it to the second one (data correctly deleted).

Summary:
Ref T5685. Currently we just 403 on an invalid token, but we can be a little more helpful.
The issues here are:
- If we **do** redirect you on this page and something goes wrong, you might get stuck in a redirect loop.
- If we **don't** redirect you, copy/pasting the link to someone (or reloading the page) gives them a pretty confusing result, since the link doesn't work any more. Prior to this diff, they get a 403.
To mitigate this, do a little better than a bare 403: give them a link to auth and generate a new URI for the file.
If this is still confusing, the next best thing I can come up with is something like this:
- Put some modulous of the timestamp in the URI.
- If the current time is within 2 seconds of the generation time, show this dialog.
- Otherwise, redirect.
That seems like it would be okay, but I worry that "2" has to be small (so links you copy/paste -> chat -> click still work) and a small value means that a small amount of clock skew breaks things. We could use the database clock, but ehhh.
Other ideas:
- Put a hash of the remote IP in the URI, redirect if it doesn't match. Fails for companies behind a NAT gateway but should work in a lot of other cases.
- Just redirect always, there's no reason it should ever loop and browsers don't really do anything bad when there's a loop (they'll show an error after too many redirects).
I'm leaning toward letting this stabilize in the wild for a bit, then trying "always redirect".

Ref T4589. When you look at a file, we load attached objects in order to run the "you can see this if you can see any attached object" policy check.
However, right now the subquery inherits the "throw on filter" flag from the parent query. This inheritance makes sense in other cases[1], but because this is an "ANY" rule it does not make sense here. In practice, it means that if the file is attached to several objects, and any of them gets filtered, you can not see the file.
Instead, explicitly drop the flag for this subquery.
[1] Sort of. It doesn't produce wrong results in other cases, but now that I think about it might produce a less-tailored error than it could. I'll look into this the next time I'm poking around.
Test Plan:
- Viewed an "All Users" file attached to a private Mock.
- Prior to this patch, I incorrectly received an exception when the Mock was loaded. This is wrong; I should be able to see the file because the policy is "All Users".

Fixes T4589. This implements much better policy behavior for files that aligns with user expectations.
Currently, all files have permissive visibility.
The new behavior is:
- Files uploaded via drag-and-drop to the home page or file upload page get permissive visibility, for ease of quickly sharing things like screenshots.
- Files uploaded via the manual file upload control get permissive visibility by default, but the user can select the policy they want at upload time in an explicit/obvious way.
- Files uploaded via drag-and-drop anywhere else (e.g., comments or Pholio) get restricted visibility (only the uploader).
- When the user applies a transaction to the object which uses the file, we attach the file to the object and punch a hole through the policies: if you can see the object, you can see the file.
- This rule requires things to use ApplicationTransactions, which is why this took so long to fix.
- The "attach stuff to the object" code has been in place for a long time and works correctly.
I'll land D8498 after this lands, too.
Test Plan:
- Uploaded via global homepage upload and file drag-and-drop upload, saw permissive visibility.
- Uploaded via comment area, saw restricted visibility.
- After commenting, verified links were established and the file became visible to users who could see the attached object.
- Verified Pholio (which is a bit of a special case) correctly attaches images.

- Maybe general tweaks.
- Maybe a placeholder (like a grey "X") for images which have been deleted.

Summary:
There are quite a few tests in Arcanist, libphutil and Phabricator that do something similar to `$this->assertEqual(false, ...)` or `$this->assertEqual(true, ...)`.
This is unnecessarily verbose and it would be cleaner if we had `assertFalse` and `assertTrue` methods.
Test Plan: I contemplated adding a unit test for the `getCallerInfo` method but wasn't sure if it was required / where it should live.

Summary:
Fixes T4550 by changing supportsFeed to shouldPublishFeedStory, so things can be more granular like that are with mail. Attempts to fix things generally too, filtering out xactions that have no business in feed, etc.
Also return an updated Task HTML representation on drag and drop moves, etc. This is important so if the priority changes you can see it reflected in the UI.
Test Plan: dragged tasks around. observed no feed stories on subpriority drags. observed feed stories and updated color bars on stories that changed priority

Summary:
Ref T1191. I believe we only have three meaningful binary fields across all applications:
- The general cache may contain gzipped content.
- The file storage blob may contain arbitrary binary content.
- The Passphrase secret can store arbitrary binary data (although it currently never does).
This adds Lisk config for binary fields, and uses `%B` where necessary.
Test Plan:
- Added and executed unit tests.
- Forced file uploads to use MySQL, uploaded binaries.
- Disabled the CONFIG_BINARY on the file storage blob and tried again, got an appropraite failure.
- Tried to register with an account containing a G-Clef, and was stopped before the insert.

Summary:
Ref T4379. I want project subscriptions to work like this (yell if this seems whacky, since it makes subscriptions mean somethign a little different for projects than they do for other objects):
- You can only subscribe to a project if you're a project member.
- When you're added as a member, you're added as a subscriber.
- When you're removed as a member, you're removed as a subscriber.
- While you're a member, you can optionally unsubscribe.
From a UI perspective:
- We don't show the subscriber list, since it's going to be some uninteresting subset of the member list.
- We don't show CC transactions in history, since they're an uninteresting near-approximation of the membership transactions.
- You only see the subscription controls if you're a member.
To do this, I've augmented `PhabricatorSubscribableInterface` with two new methods. It would be nice if we were on PHP 5.4+ and could just use traits for this, but we should get data about version usage before we think about this. For now, copy/paste the default implementations into every implementing class.
Then, I implemented the interface in `PhabricatorProject` but with alternate defaults.
Test Plan:
- Used the normal interaction on existing objects.
- This has no actual effect on projects, verified no subscription stuff mysteriously appeared.
- Hit the new error case by fiddling with the UI.

Introduce a common base class for all the workflows in the scripts in `bin/*`. This slightly reduces code duplication by moving `isExecutable()` to the base, but also provides `getViewer()`. This is a little nicer than `PhabricatorUser::getOmnipotentUser()` and gives us a layer of indirection if we ever want to introduce more general viewer mechanisms in scripts.

This provides a `phragment.getstate` and a `phragment.getpatch` Conduit method.
`phragment.getstate` - This returns the current state of the fragment and all of it's children.
`phragment.getpatch` - This accepts a base path and a mapping of paths to hashes. The mapping is for the caller to specify the current state of the files it has. This returns a list of patches that the caller needs to apply to it's files to get to the latest version.
Test Plan:
Ran the following script in a folder which had content matching a fragment and it's children:

Summary:
While we mostly have reasonable effective object accessibility when you lock a user out of an application, it's primarily enforced at the controller level. Users can still, e.g., load the handles of objects they can't actually see. Instead, lock the queries to the applications so that you can, e.g., never load a revision if you don't have access to Differential.
This has several parts:
- For PolicyAware queries, provide an application class name method.
- If the query specifies a class name and the user doesn't have permission to use it, fail the entire query unconditionally.
- For handles, simplify query construction and count all the PHIDs as "restricted" so we get a UI full of "restricted" instead of "unknown" handles.
Test Plan:
- Added a unit test to verify I got all the class names right.
- Browsed around, logged in/out as a normal user with public policies on and off.
- Browsed around, logged in/out as a restricted user with public policies on and off. With restrictions, saw all traces of restricted apps removed or restricted.

Ref T603. Several issues here:
1. Currently, `FileQuery` does not actually respect object attachment edges when doing policy checks. Everything else works fine, but this was missing an `array_keys()`.
2. Once that's fixed, we hit a bunch of recursion issues. For example, when loading a User we load the profile picture, and then that loads the User, and that loads the profile picture, etc.
3. Introduce a "Query Workspace", which holds objects we know we've loaded and know we can see but haven't finished filtering and/or attaching data to. This allows subqueries to look up objects instead of querying for them.
- We can probably generalize this a bit to make a few other queries more efficient. Pholio currently has a similar (but less general) "mock cache". However, it's keyed by ID instead of PHID so it's not easy to reuse this right now.
This is a bit complex for the problem being solved, but I think it's the cleanest approach and I believe the primitive will be useful in the future.
Test Plan: Looked at pastes, macros, mocks and projects as a logged-in and logged-out user.

Ref T603. This closes the other major policy loophole in Herald, which was that you could write a rule like:
When [Always], [Add me to CC]
...and end up getting email about everything. These rules are now enforced:
- For a //personal// rule to trigger, you must be able to see the object, and you must be able to use the application the object exists in.
- In contrast, //global// rules will //always// trigger.
Also fixes some small bugs:
- Policy control access to thumbnails was overly restrictive.
- The Pholio and Maniphest Herald rules applied only the //last// "Add CC" or "Add Project" rules, since each rule overwrote previous rules.
Test Plan:
- Created "always cc me" herald and maniphest rules with a normal user.
- Created task with "user" visibility, saw CC.
- Created task with "no one" visibility, saw no CC and error message in transcript ("user can't see the object").
- Restricted Maniphest to administrators and created a task with "user" visibility. Same deal.

Summary:
Fixes T3887. Two issues:
- Macros were generating entirely before the render cache, so audio macros worked fine in previews and the first time the cache was populated, but not afterward.
- Instead, parse them before the cache but drop them in after the cache. Clean up all the file querying, too. This makes cached remarkup generate the correct audio beahviors.
- Safari sends an HTTP request with a "Range" header, and expects a "206 Partial Content" response. If we don't give it one, it sometimes has trouble figuring out how long a piece of audio is (mostly for longer clips? Or mostly for MP3s?). I'm not exactly sure what triggers it. The net effect is that "loop" does not work when Safari gets confused. While looping a short "quack.wav" worked fine, longer MP3s didn't loop.
- Supporting "Range" and "206 Partial Content", which is straightforward, fixes this problem.
Test Plan:
- Viewed a page with lots of different cached audio macros and lots of different uncached preview audio macros, they all rendered correctly and played audio.
- Viewed a macro with a long MP3 audio loop in Safari. Verified it looped after it completed. Used Charles to check that the server received and responded to the "Range" header correctly.

Ref T3775 (discussion here). Ref T2625.
T3775 presents two problems:
# Existing tools which linked to `/differential/active/epriestley/` (that is, put a username in the URL) can't generate search links now.
# Humans can't edit the URL anymore, either.
I think (1) is an actual issue, and this fixes it. I think (2) is pretty fluff, and this doesn't really try to fix it, although it probably improves it.
The fix for (1) is:
- Provide a helper to read a parameter containing either a list of user PHIDs or a list of usernames, so `/?users[]=PHID-USER-xyz` (from a tokenizer) and `/?users=alincoln,htaft` (from an external program) are equivalent inputs.
- Rename all the form parameters to be more digestable (`authorPHIDs` -> `authors`). Almost all of them were in this form already anyway. This just gives us `?users=alincoln` instead of `userPHIDs=alincoln`.
- Inside ApplicationSearch, if a request has no query associated with it but does have query parameters, build a query from the request instead of issuing the user's default query. Basically, this means that `/differential/` runs the default query, while `/differential/?users=x` runs a custom query.

Nice title. We add three new transactions - IMAGE_FILE, IMAGE_NAME, and IMAGE_DESCRIPTION. The first is a bit like subscribers as it is a list of file phids. The latter have values of the form ($file_phid => $data), where $data is $name or $description respectively. This is because we need to collate transactions based on $file_phid...
Overall, this uses the _underyling files_ and not the "PholioImage" to determine if things are unique or not. That said, simply mark PholioImages as obsolete so inline comments about no-longer applicable PholioImages don't break.
Does a reasonable job implementing the mock. Note you can't "update" an image at this time, though you can delete and add at will.

Fixes T2691. Now, all PhabricatorActionListViews in the codebase setObjectHref to $request->getRequestURI. This value is passed over to PhabricatorActionItems right before they are rendered. If a PhabricatorActionItem is a workflow and there is no user OR the user is logged out, we used this objectURI to construct a log in URI.
Potentially added some undesirable behavior to aggressively setUser (and later setObjectURI) from within the List on Actions... This should be okay-ish unless there was a vision of actions having different user objects associated with them. I think this is a safe assumption.
Test Plan: played around with a mock all logged out (Ref T2652) and it worked!

Fixes T3143. When a user deletes a file, delete all transforms of the file too. In particular, this means that deleting an image deletes all the thumbnails of the image.
In most cases, this aligns with user expectations. The only sort of weird case I can come up with is that memes are transformations of the source macro image, so deleting grumpycat will delete all the hilarious grumpycat memes. This seems not-too-unreasonable, though, and desirable if someone accidentally uploads an inappropriate image which is promptly turned into a meme.
Test Plan:
Added a unit test which covers both inbound and outbound transformations.
Uploaded a file and deleted it, verified its thumbnail was also deleted.

Summary:
Fixes T3132. Currently, if a user deletes a file which is present in a mock, that mock throws an exception when loading. If the file is also the cover photo, the mock list throws an exception as well.
In other applications, we can sometimes deal with this (a sub-object vanishing) by implicitly hiding the parent object (for example, we can just vanish feed stories about objects which no longer exist). We can also sometimes deal with it by preventing sub-objects from being directly deleted.
However, neither approach is reasonable in this case.
If we vanish the whole mock, we'll lose all the comments and it will generally be weird. Vanishing a mock is a big deal compared to vanishing a feed story. We'll also need to load more data on the list view to prevent showing a mock on the list view and then realizing we need to vanish it on the detail view (because all of its images have been deleted).
We permit total deletion of files to allow users to recover from accidentally uploading sensitive files (which has happened a few times), and I'm hesitant to remove this capability because I think it serves a real need, so we can't prevent sub-objects from being deleted.
So we're left in a relatively unique situation. To solve this, I've added a "builtin" mechanism, which allows us to expose some resource we ship with as a PhabricatorFile. Then we just swap it out in place of the original file and proceed forward normally, as though nothing happened. The user sees a placeholder image instead of the original, but everything else works reasonably and this seems like a fairly acceptable outcome.
I believe we can use this mechanism to simplify some other code too, like default profile pictures.
Test Plan: Deleted a Pholio mock cover image's file. Implemented change, saw functional Pholio again with beautiful life-affirming "?" art replacing soul-shattering exception.

Summary:
I broke this a bit a few revisions ago by making `phabricator_render_csrf_magic()` return array instead of string without actually grepping for callsites.
Instead of building a form in JS, build it in PHP and just change its action in JS. This is simpler and doesn't require us to expose CSRF magic in more places.
This change triggered a rather subtle bug: if we rendered a normal form on the page (and thus installed the "disable forms when submitted" behavior), the download button would incorrectly disable after being clicked. This doesn't currently happen in Files because we don't put a form on the same page as the download button.
Instead, make the "disable" behavior check if the form is downloading stuff, and not disable stuff if it is. Then mark the lightbox and Files form as both download forms.
Test Plan: Used lightbox; used Files. Verified download buttons download the right stuff and behave correctly. Grepped for other download forms, but all the other places where we write "download" don't appear to actually be download links.

Summary:
- Fixes T2257. We wrote a 0-length file (erroneously?) and currently throw when retrieving it. This also happens if you intentionally upload an empty file. I'm not sure what happened with the image: we check for errors during the write, so its existence implies S3 told us the write was successful and then lost the data. Since this is a one-off, I'm not too worried about it. The indistinguishable case of an actually empty file is fixed, at least.
- Writes to a directory like "phabricator/ab/cd/efgh" instead of "phabricator/abcdefgh". When I had to go look for the file on S3 it took a few minutes of scrolling since the web interface isn't very fast. Make it so a file can be located by navigating through pieces of the hash.
Test Plan: Viewed an empty file, no fatal. Viewed the file from T2257 locally, no fatal (no data either, but it's gone). Uploaded a file, saw a nice path.

Summary:
Modernizes file uploads. In particular:
- Adds a mobile menu, with an "Upload File" item.
- Adds crumbs to the list view, detail view and upload view.
- Adds "Upload File" action to crumbs.
- Moves upload file to a separate page.
- Removes the combined upload file + recent files page.
- Makes upload file use a normal file control by default (works on mobile).
- Home page, file list and file upload page are now global drop targets which accept files dropped anywhere on them. Dragging a file into the window shows a mask and an instructional message.
- User education on this is a little weak but I think that's a big can of worms?
- Fixes a bug where dropping multiple files into a Remarkup text area produced bad results (resolves T2190).
T879 is related, although it's specifically about Maniphest. I've declined to make global drop targets yet there because there are multiple drop targets on the page with different meanings. That UI needs updating in general.
@chad, do we have an "upload" icon (counterpart to "download")?
Test Plan: Uploaded files in Maniphest, Differential, Files, and from Home. Dragged and dropped multiple files into Differential. Used crumbs, mobile.

Issues here:
- Need an application-sized "eye", or a "home" icon for "Phabricator Home".
- Some of the "apps_lb_2x" sliced images are the "_dark_" versions, not the light versions.
- If you slice an application-sized "logout" (power off) icon and application-sized "help" (questionmark in circle) icon I can replace the current menu icons and nearly get rid of "autosprite".
- To replace the icons on /applications/, the non-retina size is "4x", so we'd need "8x" for retina. Alternatively I can reduce the icon sizes by 50%.
- The "Help", "Settings" and "Logout" items currently have a "glowing" hover state, which needs a variant (or we can drop it).
- The /applications/ icons have a white hover state (or we can drop it).
- The 1x application (14x14) icons aren't used anywhere right now, should they be? Maybe in the feed in the future, etc?
- The "apps-2x" and "apps-large" sheets are the same image, but getting them to actually use the same file is a bit tricky, so I just left them separate for now.

AphrontSideNavView is an old class which required you to do a lot of work; it was obsoleted by AphrontSideNavFilterView. Remove all direct callsites so I can clean it up.
This is a precursor to letting me render a filter menu as a dropdown menu for T1960.

(eg.: Bashos' Riak CS).

Summary:
This commit doesn't change license of any file. It just makes the license implicit (inherited from LICENSE file in the root directory).
We are removing the headers for these reasons:
- It wastes space in editors, less code is visible in editor upon opening a file.
- It brings noise to diff of the first change of any file every year.
- It confuses Git file copy detection when creating small files.
- We don't have an explicit license header in other files (JS, CSS, images, documentation).
- Using license header in every file is not obligatory: http://www.apache.org/dev/apply-license.html#new.
This change is approved by Alma Chao (Lead Open Source and IP Counsel at Facebook).
Test Plan: Verified that the license survived only in LICENSE file and that it didn't modify externals.

Summary:
Minor tweaks to lightboxes.
- With "position: fixed;", we don't need to do any of the scroll/resize stuff. Just remove it.
- Make the lightbox go over the menu bar -- was it intentional that it wasn't?
- Make 'jx-mask' use "position: fixed;" too.
- Add a loading indicator.
- In Differential/Maniphest/etc, a preview may bring in an image but won't bring in the CSS we need. The "real" fix is to ship CSS/JS with ajax, but that's really hard -- fake it by pulling in the right CSS any time we render a remarkup area.
I'm going to do a couple of other tweaks here but need to update JX.Mask.
Test Plan: Verified behavior is reasonable in Safari, Firefox, Chrome with multiple images / scroll / previews / resize.
